About this opportunity
The iCare Program Specialist is responsible for ensuring appropriate and effective care coordination is being delivered by the staff in the relevant region. They oversee the rostering and management of the individual programs and is the key contact for icare coordinators and Allied Health teams to collaborate with around service delivery.
The iCare Program Specialist provides support and care coordination to people with a disability and their families under the iCare Program. The position is responsible for an overall management of iCare Program facilitation within NSW and ACT. The role includes setting up the program for the participant, end to end program support, program and business development, recruitment, ongoing guidance and support to the CDS Coordinator in the region.
The appointee will travel to various locations in NSW and ACT to engage customers as required and ensure staff are trained and to work closely with all the stakeholders in day to day running of the program and in the ongoing development and effective implementation of the program and plans as per Wellway's operational requirements.
What do you need?
- Preferred Degree in Human Services, Social Work or Behavioural Science
- Applicants having Minimum of 2 years related work experience will be at an advantage.
- A current valid Driver's License
- Satisfactory Police Records Check [less than 6 months old] and Working with Children Check, prior to commencement
- Excellent interpersonal and communication skills
